If you would like to sign up, please print and complete the form viewed above labeled "Time of Use Attachement."  Please feel free to fax this form to us at 520-424-9065 or it can be mailed to 41630 W. Louis Johnsone Dr., Maricopa, AZ 85238.  It can also be dropped by either of our locations at 41630 W. Louis Johnson Dr. or 19756 N. John Wayne Pkwy, Suite 101.
